Simple Drop-down box question--I think

I have imported a form from Word that has a drop down box.  There are around 15 lines on the form, each line is the same.  Each line contains the drop down box, and each drop down has the same choices.  The drop down box did not populate when I brought the form over from Word, so I used the Adobe drop-down box creator to create the field.  I created the first field, then copied and pasted the drop down into the rest of the lines.  Now that I have finished the form, when I select one choice from the first line, and a different choice from the drop down box in the 2nd line, ALL the drop down boxes change to be that selection from the second line (and so on and so forth...any drop down changed changes all the drop down boxes on the form).  Obviously, this isn't going to fly, so how do I get each drop-down box to behave individually?  I expect this is a really easy fix that I just don't know yet.  Thank you for your help!
