Submit button and save

I would like to make a pdf form. When the user fills out the form and presses the  "Submit" button then the form should be saved to a network location (on server) and the filename should be made of 2 form fields (  name  and  Date) plus a sequential number. Example: test25042018-1.pdf; test26042018-2; etc

Is this possible?? if yes, how to do it??

I am using Windows 10 and Adobe Acrobat DC

The server path to save to is    \\server\shared\Forms

I really would appreciate any help as I am a total beginner in javascripting.
