unable to exclude fields on submit form to e-mail and export to spreadsheet

Adobe Acrobat Pro DC 2015.006.30121 (latest version) Windows 7 Pro SP1 with 8 GB ram 308 GB free disk

I open pdf, Tools -> Prepare Form, choose Submit button in form, ctrl-I; in Button Properties, Actions tab, Actions, Mouse Up, choose Submit a form, and press Edit; in Submit Form Selections box, mailto:e-mail set, DFD Include chosen with no Comments and no Incremental changes to the PDF, under Field Selection, choose Only these... then press Select fields; in Field Selection box, choose Include Empty Fields then select Exclude Selected and try to select pilgrim_credit_card_number from the list on the left; I can not choose anything (single click, double click, right click, etc.); tried Include Selected with no results; if I choose Select All, it changes the icons on the left of each field to include a blue checkmark

I am trying to get the form data with the field name and field value into an Excel spreadsheet; I have tried each Export Format option with unsatisfactory results

any help appreciated

Whoda thought?  The manual "FIELD SELECTION Specifies what fields are returned. To receive only some of the completed field data, select Only These, click Select Fields, and select which fields to include or exclude in the Field Selection dialog box." at Setting action buttons in PDF forms, Adobe Acrobat should also say in a following sentence "Make selections by clicking on the field to highlight it and pressing the space bar."

When you can't select with the mouse cursor use the space bar. It is a bug.

For the selection use the space bar.

I have also tried FDF HTML and XDF, each with select all and deselect all, with include selected and exclude selected, with empty fields and without, using the following keystrokes: right and left click, same double click, holding control with all of the prior four.  The promise of "Field Selection : Select Fields to Submit" seems to be falling short.  The question has been unanswered for more than a year.

There is a glimmer of hope.  I was able to un-check one box.  The same box persisted from FDF to HTML to XDF. Later I was able to un-check another after the first had rechecked by returning to the prior box.

I was however never able to get two fields un-checked or discern the exact keystrokes and timing to get one un-checked.
