Unable to save completed form.

Hello,

When I attempt to modify the PDF form in either Adobe Acrobat or Adobe Reader, the entries are cleared upon saving the document.

The permissions on the form indicate that filling out form contents are allowed.

Does anyone know what the problem might be? Any help would be greatly appreciated.

That form is set up with a JavaScript that resets the form when it is saved. If you have Acrobat, you can remove this from the Document Will Save event. Otherwise, if you disable JavaScript before saving, it won't get reset when you save.
