Using a database as fields input ?

Can you use a database (CSV, XML…) as an input for the PDF forms fields, so that the end User (using Acrobat Reader) will select the row (i.e. a number) and all the fields in the form will be filled with the row's values ?

Yes, but the file needs to be embedded in the PDF itself, or be located in a pre-known location. It will require using a custom-made script, though.
