Do you mean a mail-merge? ie, producing multiple copies of the form for each record in the DB?

If so, there's no direct way of doing that with an Acrobat PDF form. If you export the data to a plain-text file, though, then you would be able to do it using some versions of Excel, or by using a script, like this one I've developed: Custom-made Adobe Scripts: Acrobat -- Mail Merge and Email
