I downloaded the form from this site and also the government site. The security in the properties section says the form should be fillable and printable. I'm using windows 7 and trying to fill it in adobe reader. I normally don't need to have an expensive program just to fill a form and not to make changes. I know the form is locked for changes.

When I print it after filling it, or save it after filling it, the form I get is blank.

So much missing information.

What computer system are you using?

What OS?

What program and version are you using to fill in the form?

What security has been applied to the PDF? Use "Ctrl" + "D" keys to access the PDF's properties.

Can you supply a link to the form?
