Please provide following information to help us identify and resolve the issue ASAP:

- Acrobat version you are using

- OS detail

- Scanner name

- Scanner driver(TWAIN/WIA) with version number

- 1 sample file

Consider using the scanner's native interface to configure the output to be black & white and output to TIFF.

Use Acrobat to create a PDF from the TIFF.
