Windows 7 Firefox No longer prompting download/save

Windows 7, FireFox current.

For years when I opened links to pdf files, I would get a prompt to download or open with reader. By mistake, I opened one the other day. Now I never get that prompt back. How do I reset this setting?

I have tried a number of things and none of have worked.

Install update and subscribe to Acrobat
Acrobat SDK and JavaScript
