Anchor points appear randomly when adding them

When I select a shape with the direct selection tool and the try to add a new anchorpoint with the pen tool, strange things happen:

A new anchor point appears, but not where I wanted it.

Any suggestions?

I'm not sure why would this happen, it works fine at my end.

Can you try doing this after setting your text display setting to 100 % and then confirm if the problem happens or not?

Legend

Select the path to modify.
Click and hold the mouse button on the Pen tool , then select the Pen tool , Add Anchor Point tool , or the Delete Anchor Point tool .
To add an anchor point, position the pointer over a path segment, and click.

Known Participant

The same result. Anchor points do not appear where I clicked with the pen tool.
