Create a Survey

hello all

I want to create a survey using AS3 on customer satisfaction with our services. There are only five questions with answer options: very good, good, good enough, not good and bad.

The survey results can be stored in XML and there is always a change when filling out surveys.

Is there a reference book or results already so that I can learn.
