Presets & Snapshots Save Incorrect or Outdated Settings

Presets and snapshots often save with the wrong or outdated settings—even when saved repeatedly—causing changes to be lost or reverted. I'm using Camera Raw 17.5, but this has been a frustrating issue for some time.
