Calendar date for x-th day of the n-th week of a month.

I know I've done this before, but my mind is a blank right now.  Head cold settling in, I think.

Can somebody get me started on the algorithm to find the date of the x-th day fo the n-th month.  I.E. 4th Thursday of November.
