CFPrint Reading custom form setup in printer driver

I'm having an issue getting cfprint to recognize the custom form I've created. I can see the paper attribute has a custom option but I'm not seeing any way to get those values to change.

Anyone have any insight on getting custom forms/paper sizes to carry over into cfprint?
