Flash Forms

I am having difficulty getting Flash Forms to display anything on the page. I was getting a JavaScript error, and added a virtual directory mapping to CFIDE and that error message is now gone, however nothing is displaying. I have a more advanced form I am trying to get to work, but for now I am just working on getting something very simple to display so I can move on from there. Any thoughts as to what might be off?
