LDAP Connection Help

I have been trying to connect to my LDAP server and no matter what I do, it won't work. When I use a LDAP browser I can connect, but not not bind. When I use the script attached, I simply get a "Connection to LDAP server failed" I really have no experience with LDAP and only some with AD. Nobody else in my office knows much about LDAP, so any help would be greatly appreciated.
