Menu bar issue in CS4

THERE'S AN ELEPHANT IN THE ROOM:

CS4 hasn't been updated since it came out in 2008.  And nobody should be using outdated web authoring software, too much has changed.  Switch to a modern code editor, if not Dreamweaver CC then something else.

If clearing cache doesn't work you can try re-installing.
