Need to use a database driven app?

I design web sites, when it comes to log in pages, comment pages I'm a bit lost. I have a client who needs to have two pages within her site/

First - a page where registered users can leave comments on a page.

Second - A log in page where you would have to log in to the site to leave a comment.

I have a pic of the exact way client wants this. See url with jpg.

My question is : I know the e mail and comment page is .php but what about the log in page is it my sql or asp.net? If so, I design on a Mac am I going to have to use a MS driven database? I know they make My SQL for OS X but as I said I don't know and need to find out what to do here. Anybody??? database needs to function like this

Why does it need to be a MS driven database? Why not MySql? MySql can run
on Microsoft based servers.
