Web Application Help

I am completely lost and need help...I am using Dreamweaver CS3, and I am trying to create a page where an end user fills in text boxes and confirms their email address (with an email being sent to them confirming that it is accurate). I want to be able to store this information directly into a database. Are there any tutorials available that will help me get started? Any help would be greatly appreciated.

Open the index page, change the content, and save it with a new name?

I am now having another issue. The tutorial was GREAT I learned a lot, but I am stuck in another situation. I need to get dreamweaver to connect to the MySQL database located where the domain is being kept I am not sure of how to do this at all. Any help would be greatly appreciated.
