CS4 prompts for License Agreement

All my CS4 products open with a License Agreement screen.  Clicking Accept does not work.  This product has been activated previously.

I did try Captivate 9 and did not buy it.  I am wondering if cancelling it triggered this.  Everything had been working OK for years.
