Constant Flash Crashing -Help!

I have tired un-installing and re-installing flash and it has not help. Keep getting "plug in failures" , large exclamation marks with loud buzzing sound,  also

caused my computer to freeze and can not click on anything.This has been happening for over a month. Apple store has also reset my Mac to original factory setting and still having this problem not as bad as before but its slowing starting up again.

I have a Mac OS X (MacBook Pro)

El Capitan Verison 10.11.4  but had Yosemite in the past with same problem

Safari 9.1

NPAPI Plug in Version 21.0.0.226

PPAPI Plug in Version 21.0.0.213 installed

If anyone can help it would be greatly appreciated .

    The exclamation point icon means that either Flash Player has run out of memory (which is typically a symptom of buggy content), or we've arrived in a situation where we think a particular memory allocation is unsafe to make (in which case, we just use the out of memory mechanism to exit in a quick and safe way). 

    The buzzing sound indicates a corrupted audio buffer, which could be at the heart of why we're throwing the OOM icon.

    A link to the website where you usually run into this would be really helpful, and if you have a set of steps that consistently makes it happen, that would be really, really helpful.

    Is there a test to see if my website contains flash software? I also have a mac, but with Yosemite version 10.10.5., and my browser crashes sometimes when I visit.
