Have to reinstall on restart every time

Macbook Pro running Sierra.

Each time I restart my Macbook I am asked to reinstall Flash player and a window opens up (see below). It then says: "Install Adobe Flash Player on the disk image. AdobeFLashPlayer_19_a_install.dmg" Safari downloaded this disk image on 28 Oct 2015." Options then are to cancel, Show disk image and open

Each time I open this and go through a three stage install.

I use both chrome and safari.

Any ideas how to avoid this?

    It's possible you have a mounted disk image that's hidden.  Please do the following:

    • using Spotlight search for Disk Utility and open it
    • In the left panel look for any mounted Flash Player disk image
    • If it's there, unmount the disk image
