Challenge Viewing Photos InCopy

In Incopy when viewing the document, if a duplicate headshot is being used next to a class description, the headshot is showing as a box that is grayed out and when printing inhouse on printer, the gray box prints versus actual photo - any suggestions?

We have checked our viewer display performance settings and on typical or high quality display still grayed out.
