Problems with InCopy Track Changes

I have a problem that I can't seem to resolve. We have a publishing team that is using InCopy for editing articles. Each user has their own color so we can see who has done what to the article. All these show up for me, but one. One user who's color is yellow, shows up as my color and it also says I made those changes (which I did not). I've upgraded my InCopy. I've upgraded his InCopy (15.0.2). I changed my color. What am I missing?

A related issue is that we change the tracking color for a different user and no mater what color we change it to, it comes up as green. What are we missing?
