Replacement Photos in Layout Not Updating in InCopy

When I open my icma files in InCopy to check the designer's layouts, I'm bumping into a problem -- specifically with photos. On a couple of occasions, the designer has had to change a photo in the layout. She claims the images are displaying correctly in her InDesign file; however, I can only see the old photos, not the replacements in InCopy. The text portions of the layout seem to be behaving and updating appropriately. Why would certain photos not be updating in InCopy? What should we be checking to fix this?
