f4v recording problems

We have been recording the f4v files for months using flash media live encoder and its worked well, however all of a sudden for me and another user i know the visual has started automatically splitting into 3 and when played it stops after a few minutes.

Have you any idea why this might be and what we can do to rectify it and stop it happening? Hope you can help, its driving us mad!
