How do I get Mp4 files to play in FMS?

I have the FMS working with .flv files but I can't seem to get .mp4 or any other format file to play using it?

Are you by chance using sub-folders under the VOD app? That was the mistake I made. Example:

Wrong:

http://testserver.com/vod/department/mp4:video.f4v.

Correct:

http://testserver.com/vod/mp4:department/video.f4v.
