Auto fill based on name drop down menu

I have a fillable form I made. At the bottom I need it to auto fill based on a name.  

 

I have a drop down list of 14 names

 

I need the age, Address, and phone number to be auto filled based on the name selected. 

 

How do i do this.  I know I need a mast list somewhere that way it knows the data to enter. Where is this put. I made it in excel for now. 

 

Some one please help me with the script I have to enter.  This is whooping me

Actually, I know for a fact that I and many other frequent posters have answered this exact question many times. And many times recently. In fact, I wrote articles on this topic over a decade ago.  It may be that the asking overwhelms the answering 😉

So the question is, where did you go wrong? What have you done to debug?  Why don't you post your code and make sure to say where it was entered.

In addition to Thom's excellent tutorials (and the many posts here on the subject), I also wrote a script that does it for you, so you don't need to write any code yourself. Just run it on your spreadsheet (after saving it as a text file), and it takes care of everything.
