Button in PDF form does not work on MacOS Mojave

I'm trying to fill out this form using the latest version of Acrobat Reader on MacOS Mojave:

The buttons in the PDF don't work.  The "Add" button is supposed to add a row to the table, but it just flashes yellow and does nothing.

 

Why doesn't this work?

What exact version of Reader are you using? Did you open the file in any other application beside Reader?

Yes, I must have opened in Mac Preview before using Reader.  Downloading a new copy and opening in Reader works.

 

Huge fail on Adobe's part for not at least providing a decent error message in this situation.  😞  No wonder that Mac users prefer Preview and only use Reader when they absolutely have to...

So it's Adobe's fault that Apple created an application that corrupts PDF files just by opening them? Some twisted logic there, buddy. And it's not only LCD forms, by the way.
