I am creating a document with approximately 25 questions that my patients will fill out. The questions will be answered by selecting answers from items in a checklist or drop down menu. Is it possible to create some code to have the selected items be listed in a summary section at the end of the document? Any information would be greatly appreciated

Yes, it is. Do you just want to aggregate them into a single text field, basically?
What should happen if no answer is selected in some fields?

I appreciate you responding to my post. I have questions that all have answers even if the answer is....Non-applicable. I placed a screen shot of one of the test pages. These are 4 of the 25 diagnosis points that I go through and answer. My goal would be to compile all of the answers into a section of the pdf at the end as a summary or even in a whole separate document so that things are clean and concise for the patient to read. See the problem is that you can't hide all the choices to each question in a drop down and have multiple selections. Sometimes I might have two or three of the items for each question selected and you can't do that with a drop down therefore the choices to the questions have to be listed in each section as a checklist which to me is overwhelming to present to the patient. Let me know your thoughts

All of this is possible, although copying the data to another file is a bit tricky, especially automatically. But creating a sort of "summary" page with just the user's selections is certainly doable.
However, this would require the development of a custom-made script. If you're interested in hiring a professional to create it for you, feel free to contact me privately by clicking my user-name and then on "Send a Message".
