converting a PDF to HTML with all text fields, checkboxes etc

I want to be able to create html code and the pdf will be the background of the page.
1. I imported an image file into a pdf in order to have a background for the pdf form
2. Then I switched to form mode and placed some so-called text fields on the pdf above which there is an image background.
3. Finally, I thought I could create a web page on which there are the text fields with the locations and the background of the page will be the original pdf document
4. What I saw in the html code is just the background image without the fields.

 

how can I generate html web page with all the fields ?

Acrobat can't do this. Maybe there is a 3rd party solution, but I doubt it.  This is not a feature a lot of people are asking about.
