Hi, please be patient with me!
I have Adobe Acrobat Pro, and I am used to making Fillable Forms simply by having something to the effect of:
Patient Name: ___________
Tell me about yourself: (insert a fillable box here)
But I have this form now, that I need to make fillable and easy for our therapists. Obviously the areas that have [ ] or ( ) need to be fillable. How do I simply make it to where they can click that [ ] space or ( ) space and fill in the information needed? I feel so silly!

There's no simple way of doing that with a PDF file, especially not with such long fields. You will have to leave empty spaces for the areas where you want the user to fill in long text and then add multi-line text fields in those areas. Make sure to leave a rectangular area for it, though. A field can't span over one and a half lines...
It's also not the best idea to have a text field in the middle of a paragraph, as it would require to set the font text to Auto (to accommodate for longer texts), making it out of sync with the rest of the text in the sentence.
