Copying Checkbox FIelds In Form

I have a fillable form in which I need to provide 574 unique checkboxes.

That is a lot to manually enter on by one, so I created one column of 41 checkboxes, and then copied those 41 checkboxes as a group to the remaining 13 columns.

But what I'm finding now though, is that when I check a check box, all the boxes populate. 
I didn't realize that copying a series of checkboxes would result in this behaviour. Is this not possible? Or is there just a parameter I missed? They were all uniquely named with the same textual prefix I created for the first box, and then each has a sequentially assigned number., 

Or do I need to do all 574 boxes individually?

Use unique names for the checkboxes.

Select prepare form tool and right click on checkbox and select 'Create multiple copies', this will create checkboxes with unique names.
