drop down menu add additional fields

I'm trying to get additional fields created when the number is selected in the drop down menu:

when the number is selected it will create additional fields for this:

Can someone help me with this?

If the forms are to be used in the free Reader, one cannot add fields. You also need to realize that fields must have unique names unless you want the added fields to share the same value as an existing field.

You might want to look at the use of templates to spawn new pages as needed. Newer versions of Reader can spawn new pages from templates.

One could also use hidden fields and then make them visible as needed.
