When I combine 16 PDF files into one PDF, all of the pages are duplicates of the first file. How can I resolve this issue and have one copy of each page?

Are you referring to the pages themselves, or to the form fields in them? If the latter, that will happen when you have fields with the same name in the source files. The solution is to either rename the fields before merging the files, or flatten the fields before merging, or merge the files as a PDF Portfolio.

Hope you are doing well and sorry for the trouble. As described on combining the PDF the pages of the first file gets duplicated.
Is this a behavior with a particular PDF file or with all the PDF that you try to combine? Please try with a different set of PDF files and check.
What is the version of the Adobe Acrobat DC you are using to check the version go to Help > About Acrobat and make sure you have the latest version 21.07.20091 installed. Go to Help > Check for updates and reboot the computer once.
Also try to reset the Acrobat preferences as described in the help page https://community.adobe.com/t5/acrobat-discussions/how-to-reset-acrobat-preference-settings-to-defau...
You may also try to create a new test user profile with full admin rights in Windows or enable the root account in Mac and try using the application there and check.
