Editing a PDF

I am trying to edit a pdf - when I change the text in one box, it changes the text in a bunch of other boxes. I don't understand the connection and how to break the connection between these boxes. I need it to change only one box of text.

The fields have the same name, and therefore the same value. They each need

to have a unique name.
