Has anyone received this message? What are the "Additional files"? I had some issues with this a while back but thought everything was fixed and now this....

Ok. Please open Word and see if the ACROBAT ribbon appears along the top. If so, please convert to PDF and try again. If not, we need to fix that.

Hey Angelab,
Sorry to see that you are struggling with the same issue again.
Looking at the screenshot shared above, it seems you are creating form from one of the PDF saved locally on your computer, is that correct? If not, then try saving the PDF locally first and then select "Prepare Form" to continue.
To troubleshoot and get over from this error, you may perform these steps-

I ran the cleaner tool and reinstalled. Still having the same issue though.

What is the file (customer survey 7) you are starting with? For example, a Word document?
