Can Acrobat create a fillable form from an Excel file, including form controls, ActiveX controls, and text manipulation formulas?

ActiveX controls are not supported in PDF files.

It's not a straight line from Excel form to PDF form.  They are completely different animals. You're better off designing and building the PDF form from scratch.
