I need a blank template I can fill out and save then reuse the blank template, how do I do that

I scanned a  blank entry form that I can fill out and save.  I have 25 hand written entry forms and I need a blank template for each one , after filling the form how do I   save it and  then get the blank form back, does anyone know how to do this?

I am helping a friend make a catalog from hand written entry forms.  I can scan a   form  and edit it to use as a templates for each category,  How do I save the template for multiple uses.  After filling out the form where do I save that completed form?  AND I still need the blank template document. that I can use to put in the information from the hand written entries.

I hope you can tell what I am trying to do.

I have a form I can scan and edit for each category

How do I save the template for each category and where do I save it

After completing a form how do I save the complete form, and still have the blank document for the next entry@

Thanks for your help...I am VERY new at this and I am getting frustrated.

Create your blank form (by the way, you should really use form fields instead of doing it by hand). Set it as Read-Only (in Windows Explorer). Now open it and fill it in. At the end of the process you won't be able to overwrite the original file (since it's read-only), and will be forced to save it under a new name. Once you've done that close the file and go back to the blank, original form, and start over.
