Image and Date field in Forms

Hi,

I'm creating a Form but the date field is shown as text. Also I don't see Image Field.

Any suggestions?

Click the 3 dots to the right of Prepare Form and select "Remove e-signing options".  Next time don't select "this document requires signatues" when "Prepare a form" is activated.
