Have you opened this document in Adobe Acrobat or Reader desktop application?

This looks like a scanned PDF. Have you been trying to edit the content?

If its a scanned file and doesn't contain real text, then while editing the document, it will show this way. If you want to edit the text, click inside the box and start editing.

There's likely a chance that you would not get the accurate result because of the type of file it is. If this would have been a fillable form, then editing would have been a different experience.
