I'm having issues with a form, of which I am the original author. I was asked to update the forms by adding some content, but when I got to save the form, a lot of the text in the form all of the sudden is replaced with blocks. Screenshot is attached. Does anyone know why this would occur?

It's occurring in both form fields, and the text of the document. As far as I know this was not a scanned form, it was an existing pdf document created a long time ago. I was then given the document to convert to an interactive form so that the form could be completed electronically. I've had no problem in the past revising this form, when need be, up until this last time where upon saving the document these boxes appeared all over the form. I've tried it again several times with other pdf documents, and the same thing occurs. As far as using OCR, I'm sure I did when I first created this form, and I would've used the default settings for that. Below is the properties on one of the form fields you had asked to see.
