mutlipage document using templates and forms

I'm vaguely familiar with how templates work, but I'm trying to create a mult-page intake form. I'm an insurance agent and want a document where I have a main page with mutliple checkboxes (auto, home, motorcycle, etc) that will spawn intake pages for only the ones checked. Some of the field names need to be the same across many pages (like name and address) so they autopopulate. My last attempt at this created new field names titled with P1..... and P2... I'm assuming because it was spawned as a different page number each time. Is this possible and am I on the right track?
