pdf form

i update a contract in word and saved it as a adobe pdf 

i open it up in adobe pro that now looks different. and did the prepare form i set all of the fill line to center added 40 check boxes preview looked good did a test run to request e sig. the boxes did not send checked 

and all of the fill wa moved to the left any ideas
