I am trying to create a form for our website that collects patient info, as well as the parents signature (that will be the next hurdle) but when I click "Prepare Form", the checkbox and option just under the document image is missing. This is the line that says "This document requires signatures". It just isn't there, and I think I need it to make this work. I see it in every online video I have looked at. Can someone tell me how to get it back?
I am using Acrobat Professional DC
I am on a Windows 10 machine

Sorry for the delay in response.
You may open the form in Acrobat, click on File>Properties>Security. Check if you are allowed to modify the document?
Also, check if you are experiencing the same issue with a new form? Ideally, this shouldn't unless you are using the classic version of Acrobat DC. Still, you should be able to see the checkbox field option. Also, help us with the version of Acrobat installed on your machine, launch Acrobat>Help>About Adobe Acrobat.

Hi Tariq, I'm having the same issue.
The "This document requires Signature" checkbox is not there when I select a document to Prepare Form.
I am using Adobe Acrobat Pro DC - 2015 Release (Classic) Version 2015.006.30352. I cannot use a different version as this was purchased and installed by my company. I cannot upgrade until our central IT upgrades everyone.
I have permission to modify the document, and the checkbox is not there whether I am creating a new form or trying to modify an existing form.
In preferences, under Digital Signatures, I have defined the appearance of a signature block. However, when I insert a signature block into a document, it does not use the settings I defined. In my form editor I can see the signature block, but it just appears as a standard text box to the recipients of the document.
I can insert a Signature Field, a Signature Block, or an Initial Field, but all just appear as text entries on the final form.
Am I stuck with text only forms with this version, and if so, then why all the signature options if none of them work?

I am trying to build a form from scratch and searching the internet all i get for an answer is to select tools/create but when i select that i get a dialog box that gives me the option to use a current document or scan a paper form. When i select use the current form it tells me to select a file to open but i don't have one yet, and obviously i wouldn't select scan so how on earth can i create a new form ?

You can convert a empty Word document to PDF and use this PDF file.
Or use Adobe Acrobat DC.

Acrobat is not built for creating forms from scratch. You need to have a static layout already created, and then add the form fields to it. In your version that would be using Tools - Forms - Edit.

Just make an empty form in word and save it as a pdf and use that.
