Send client webform to client not creator email address

I am creating a webform for a Doctor. I want the results of the form sent only to the doctor and not to me which would be a violation to the patient. How can I have the form submitted ONLY to my doctor client.

The Dr will need one or the other subscription. I don't know if Adobe sign is enough, but as Adobe is not a non-profit organisation, I won't think that the doctor can get a for free access to Adobe sign. I would also think that the price for the subscription depends on the number of forms that get sent out. And for sure, the doctor's account needs to send out the forms.

PDF forms are not really web forms.  How exactly are you creating this form? And how do you expect it to be used?

Thank you for your willingness to help.

I am using the adobe "create a form" pictured below, to create a link to post the form, on my clients website. I created the fillable form in Acropbat Pro. I want the patients to be able to fill in in online and submit it to the doctor's email, and NOT my email address.

This is not really the correct forum or this question. What you are talking about is an Adobe Sign form. The responses are sent to the owner of the Adobe Sign Account from which the forms are sent. When you purchase Acrobat Pro you get a free Adobe Sign account. 

For the Dr to get the responses he'll need to have an account and send the forms from that account. You can still build the forms, but they'll need to be transfered to him.

Hello,

Actually I worked with tech support at Adobe today and learned that the form I am using is not Adobe Sign because I am having the end user fill out a form. Adobe Sign is only for signing completed documents. The only way I could have the result sent to the doctor is to CC her on the form reply but that doesn't work because I still receive a copy of the completed form.

 

Perhaps she can get an adobe acount without purchasing Acrobat Pro as you suggest. I will look into that.

Adobe support is famous for providing bad to completely fantasy based advice. The screen shot in the first post of the service you are using, is Adobe  Sign. 

True PDF forms cannot be filled out online. They must be filled on the client system using a PDF viewer. The dynamics of this can be confusing and very problematic for reasons I won't get into. The big advantage of PDF forms is that they are easy to create. Usability is a different issue. 

The Adobe Sign service is a way to bridge the divide between PDF forms and Web forms, it's not just about signing. You create a PDF form and submit it to the service, where it is converted into a web form that is presented to the recipient(s).  Form routing and data handling are built into the service. 

 

However, if you'd like to use a pure PDF form, than can be done as well. But it is not a web form. PDF forms are handled very differently from web forms.

Do you mean the "Adobe Form builder"?  Again, this is Adobe Sign.  After a form is submitted to the Adobe Sign service it is converted to a web form and stored in your account's list of available forms.  Form Builder is just a way to do this. 

The service provides a way to make a link to a an available form  that can be put on a web site. Anyone who clicks on the link can fill out and submit the form. The submitted form is saved back to your Adobe account where it can be accessed.
