Trying to add an menu action on a button but menu actions list does not show anymore.

I was using Acrobat to create fillable forms. Its a breeze with it and great options. I have successfully create a few forms for our HR group. I was creating yet another one ( in prepare form) and about to add an action to a button when I noticed that the "execute menu item" choices dialogue box was empty. It was populated last week and not is empty. Is there anyway I can repair or troubleshoot it so the choices are populated again?

Could you please confirm the version of Acrobat running on the machine.

To check the version, launch Acrobat>click Help>About Adobe Acrobat

Also, try preparing another form in Acrobat and see if Execute a menu item options are there.

Thanks for the reply. I am using Adobe Acrobat PRO DC version 2015.023.20056.
