I am trying to create an Adobe Form in which I can have one of the multiple signatures be different each time I use it.
Basically we have a standard form for students that is used for a specific scenario. Each time we use it, four team members "touch and sign" the form electronically each time, but the student changes each time.
RIght now, we open the form every time, then add the "form filler" and the remaining four signatures, then go into the form and apply each signature to the correct fields, then "send".
What we would like is when we open the form, all the team signatures are there and the proper fields they complete attached. All we would need to do is add the student email and name, then attach the proper fields to them.
Basically can I save a FORM with all the emails already in it, then open it and add the remaining student email for signature?

Are you referring to a PDF form with digital signature fields?

Yes. I created the PDF first, the brought into Adobe Pro and created the form.
Then I load that "form" into Adobe Pro (online) and select "Request eSignatures". We then go through the process of entering all the emails and names of the folks who will sign, starting with the "Form Filler" then adding the recipients.
Once done, it goes to Prepare Document where we can tell Adobe which fields get filled out by which Recipients, starting with the Form Filler who enters most of the data. Each recipient is limited to specific fields THEY must complete.
My point is that the FORM FILLER is always the same email address, and three of the four recipients are also the same people/emails and all complete the same fields (but those fields are data that is entered, not static info). The only recipient that changes each time is the "student". They need to check "Acknowledgment" boxes and sign the document.
So, the question is, can I set this up so everything the Form Filler and the three staff member recipients, stay the same each time, with the same fields assigned to them, and just change the student info (email and name)?

Thank you for reaching out, and sorry about the delay in response.
In Acrobat Sign, when you send the document for signature for the first time, adding all the form fields, you can save it as a template before sending it for signature. On the page where you add the form field in Acrobat Sign, please click on Save before the Send option.
Please refer to the information in the following help document: https://helpx.adobe.com/sign/using/manage/create-template-from-agreement.html.
Or you can create a template and assign the form fields as Participant 1, Participant 2, Participant 3, and Participant 4. When you use the template to send for signature, you will just need to add the email addresses of the recipients in the order you have added the participants to the template. For example, you have added form fields for a student in the template as Participant 1, then add the student's email address as Recipient 1.
However, it is not possible to add the email address of recipients in advance. You may assign the form fields as participant numbers. So, when you add the email address in the recipient field, the respective participant number fields will automatically assigned to that recipient.
Let us know if you have any questions or need any help.
Thanks,
