Downloaded and installed the add in " Create Bookmark Report".
From the following site: https://acrobatusers.com/actions-exchange?trackingid=JZXYQ
Local machine being used runs most current updated Windows 10 pro and using Adobe Acrobat XI Pro
But I don't get a report.
Something I maybe missing.
1st screen shot of add in being used:
(there are number of bookmarks in the Bookmarks 2, 3, & 4)
2nd screen shot of report but no bookmarks- also wanted a csv report but none available:

I don't know this script and it's too complicated to look at, but perhaps there is something simple you are doing that isn't expected. When you run the script it should show a dialog box with questions. What happens? Why do you say that no CSV report is available - does that choice not appear?

Does happens also with other PDF files?

Here is what's supposed to happen when you start this action:
As was already mentioned, you should see a dialog:
There is a checkbox that indicates whether you want to create a CSV version of the report (instead of just the report in PDF form). Once you start the process by clicking on OK, two files will be generated: A file named BookmarkSummary_Bookmarks.pdf, which contains a summary about the operation, this file will be updated if you run more bookmarks summaries in the same folder.
In addition to that, you will also find a file that is named <originalFileName>_Bookmarks.pdf (or whatever postfix you've specified on the dialog). This file will show the bookmarks, and it will - if configured to do so - contain the CSV data as an attachment:
You need to open the attachment (you can either double-click on it, or right-click and select to open or save the attachment). If you see something different, please be as specific as possible.
