G'Day All,

I am creating a fillable form for our daily work sheet.

We give each form a unique number so that they never double up. 

I want to use the initials of the Supervisors name and the date the form was signed to create the unique number.

Is this even possible?

Is there a field that holds the supervisor's name? Is the form signed digitally?

What should happen if the same supervisor signs two forms in a single day? You would get duplicate numbers. You should maybe include a full time-stamp, not just the day, or a random number.
